Please fill out the questionnaire for the Divine Liturgy you wish to attend. Submissions are to be filled out on a weekly basis and will be cleared each week.

To be pre-registered for one of the weekend Divine Liturgies, it is recommended that your response is submitted by the preceding Friday at 5:00 pm in order to be included for the Saturday and Sunday services, since the Parish Office is closed on the weekend.
